BILLING POLICY

Insurance Requirements
Please be aware that different insurance companies provide coverage for physical therapy services, but may require prior authorization from your primary care physician. We can assist you if you have questions regarding pre-authorization. Please bring your insurance card and physician's referral when you come to your first appointment.

Deductibles/Co-payments
Your insurance may require a deductible, or co-payment. These payments are required at the time of treatment. Payment is accepted in cash or check made out to Manual Physical Therapy & Sports Medicine (MPT & SM). A statement will be mailed to you for any balance due to MPT & SM following insurance processing according to the policy and contractual obligations of the insurance company. Payment is expected upon receipt of services. Should there be any questions or concerns regarding your statement, please call our billing department at the number provided on your statement.

Worker's Compensation/Automobile Accident
If this is a workman's compensation or automobile accident, please bring claim number, the Adjuster's name and phone number, and any other pertinent billing information. Patients who receive worker's compensation benefits are advised that they may lose their benefits if they miss therapy or abuse cancellations.
